Responsible for managing and supervising all aspects of nursing/clinical support operations, including ensuring quality patient care and regulatory compliance. Key duties include managing staff through hiring, training, and scheduling; overseeing the developing and implementing policies and procedures; and acting as a liaison between the nursing staff, physicians, and administration.

  • Establishes and guides Quality Improvement teams to address priorities for improvement activities. Works in partnership to oversee data collection for quality management, statistics and reports for evaluating Agency performance.
  • Ensure the clinical department complies with all local, state, and federal healthcare regulations and standards.
  • Sets and tracks departmental goals and objectives.
  • Lead quality improvement initiatives and ensure high-quality patient care
  • Develop, implement, and update policies and procedures to improve operations and patient care.
  • Oversees assigned staff to ensure efficient clinic workflow; streamline workflow processes, scheduling and proper coverage and effectively oversee the management of CCHC's clinics.
  • Designs and manage continuous quality improvement (CQI) initiatives.
  • Ensures that all guidelines, regulations, and standards are followed.
  • Work closely with Director of Risk Management and Quality Services to implement and track clinical operational efficiency with PCMH & FTCA Standards.
  • Supervise and mentor on-site clinical support care staff, increasing our coordination of care across departments, enhancing workflows for patient care coordination, enriching nurse education visits and improving standard of care and patient satisfaction.
  • Collaborate with the Medical Director to integrate clinical disciplines and optimize all team members to meet quality improvement goals.
  • Develop and implement care coordination in accordance with FQHC, PCMH, Blueprint for Health and ACO standards, values and models
  • Lead practice transformation and improvement efforts to meet ACO goals and collaborate with multidisciplinary teams in developing plans for achieving success in ACO quality metrics.
  • Monitors Nurses respond to triage calls in person and/or on the nurse phone line directs them to emergency rooms (ERs), urgent care centers, or to schedule patients to their physician during office hours.
  • Develop a positive culture to promote continuous growth and staff development, establish best practices and the education and support systems to sustain them
  • Provide supervision and mentorship to Medical Assistants, LPNS and RNs in our health centers supporting responsibilities including but not limited to triage, rooming, medication prescription refill management, Care Coordination, immunizations, infection control, employee health, preventative health education and promotion.
  • Be a champion for quality improvement and innovation, encourage and lead QI projects, evaluate workflows, develop innovations and participate in data reviews and learning collaboratives.

  • Conducts Clinical supervision of LPNs and MAs; ensure quality services are provided to the community and overall compliance with the State of IL Nurse Practice Act.
  • Manages and coordinates the Vaccines for Children (VFC) program and private stock, including, but not limited to, the following: oversees, implements, monitors and ensures compliance with requirements; conducts regular (weekly/monthly) internal audits; prepares for external audits; implements/coordinates staff training; ensures proper documentation; completes timely internal/external reporting; and attends VFC external training sessions with identified clinical care team members.

  • BS degree in Nursing.
  • Current unrestricted IL RN License.
  • At least 5 years professional nursing and clinical leadership experience; preferably in a community health clinic setting.
  • Knowledge of and experience with EMR and Microsoft Office systems required.
  • Proven understanding with clinical protocols, performance measures and patient health outcomes;
  • Must possess strong organizational, administrative and time management skills;
  • Must possess strong verbal, computer, and communication skills;
  • Must have vehicle to travel between clinic sites and external meetings.
  • Experience with Patient Centered Medical Home model and concepts preferred.
  • Experience with community health center and federally qualified health center (FQHC) services/models strongly preferred;
