We are seeking a high-performing Director of Nursing (DON) to lead clinical operations for our Medicare-certified home health agency. This is a role for a true clinician-leader—one who understands home health from the ground up and wants to build an educated and successful clinical team.
Requirements
- DON, ADON, Clinical Manager, or Quality Lead experience in Medicare home health
- 5+ years of full-time Case Managing RN experience in home health
- Deep knowledge of OASIS, PDGM, Medicare Conditions of Participation, and Colorado regulations
- Proven ability to support, coach, and elevate field clinicians
- EXCELLENT organizational, communication, and technology skills
- Active Colorado RN license (BSN or MSN preferred)
Responsibilities
- Lead and mentor all clinical staff to deliver exceptional care
- Ensure full regulatory compliance and clinical documentation excellence
- Oversee OASIS accuracy, care planning, and patient-centered workflows
- Drive quality outcomes and performance improvement initiatives
- Support recruiting, onboarding, and development of clinicians
- Work hand-in-hand with an executive team that values your expertise and supports your success
