About The Position

Provides operational and clinical leadership to PPCH Nursing Services to ensure quality and inclusive health care and care coordination for customers with intellectual or developmental disabilities (I/DD). Oversees the development, implementation, and evaluation of care plans in a community setting, while working with other health care providers and agencies to integrate care.

Requirements

  • Colorado Registered Nurse (RN) license in good standing required.
  • Minimum of 1+ years’ experience working with people with I/DD required.
  • Proficient using Microsoft Office suite or other similar software required.
  • Excellent written and verbal communication skills.
  • Effective listening skills, even when non-traditional communication is used.
  • Ability to direct others.
  • Ability to respectfully address significant issues of health and safety while supporting an individual’s choices.
  • Knowledge of the Colorado Developmental Disabilities system, regulations, and rules required.
  • Skill and ability to constructively receive and provide feedback.
  • Ability and willingness to complete required trainings and learn PPCH operations and systems.
  • Valid driver’s license, and ability to meet PPCH’s driving requirements.

  • Developmental Disabilities Certification (DDC) preferred.

Responsibilities

  • Oversees the health-related services and care plans for PPCH customers.
  • Ensures that services are provided in a manner that respects and acknowledges individuals’ self-expression, self-worth, self-reliance, and decision making.
  • Hires, supervises, and leads Nursing Services staff.
  • Process physician orders on Medication Administration Record (MAR), reviews MARs for accuracy, and updates MARs as needed.
  • Works continuously to improve services and supports using a person-centered approach.
  • Recommends updated protocols, staff and provider trainings, or other changes when areas for improvement in the provision of care are found.
  • In an on-call rotation with the Nursing Team, provides 24-hour telephone support to caregivers and staff.
  • Provides medical care and meets administrative requirements for customers on caseload.
  • Attends and participates in team meetings for customers on caseload, and revises medical plans as needed.
  • Develops and maintains comprehensive Care Plans for customers on caseload.
  • Completes home or hospital visits with customers.
  • Contributes to a culture where individuals receiving services are healthy, safe, and valued by others.
  • Models, teaches, and ensures respect for others, including individuals in services, staff, other professionals, guardians and loved ones, and team members.
  • Provides client specific medical training to PPCH contractors and staff.
  • Responsible for providing QMAP training and monitoring for staff and contractors.
  • Responsible for G-tube training and monitoring for qualified staff.
  • Provides information, support, and guidance to community members, other medical professionals, contractors, and staff regarding the specific needs of clients.
  • Promptly notes and addresses gaps in skills, understanding, or compliance with staff and contractors.
  • Communicates regularly with Program Directors, guardians, medical providers, and team members to ensure the customer’s health and wellness needs are met.
  • Strives for full compliance with PPCH, State, Federal, and other applicable guidelines and regulations.
  • Ensures that Nursing Services staff have completed required training, including annual refreshers, according to established timelines.
  • Monitors, completes, and provides required documentation in a timely and accurate manner.
  • Promptly communicates and reports all behavioral incidents, medical emergencies, and/or significant incidents to appropriate agencies, teams, or individuals.
  • Maintains knowledge and understanding of PPCH and other regulating agencies’ policies, procedures, rules, regulations, and guidelines.
  • Assists with internal or external audits as needed.
  • May provide employee tuberculosis testing and Hepatitis B vaccinations.
  • Other duties as assigned.
