Be able to lead the clinical team with a sense of purpose and accountability; utilize metrics and established goals. Oversees the medication management services, including training and audit of medication administration practices, collaboration with prescribing healthcare providers and pharmacy staff Identifies ongoing needs and services of residents through assessment and service plans utilizing the nursing process Visit prospective/current residents in their homes or healthcare settings as needed to assess for admission/readmission to the community. Clinical liaison with families, residents, healthcare professionals, and team members Direct the infection control measures for the community Work collaboratively with others on the community leadership team Manage and coordinate the work of the clinical staff. This includes overseeing and/or completing required training, staff schedules monthly and daily work assignments. Provide on-call clinical support as needed.
