About The Position
The position involves conducting initial assessments of prospective residents, including off-site visits, and performing wellness assessments to develop individualized care plans based on the needs and preferences of each resident. The role includes delivering health promotion services, offering disease prevention counseling, and providing self-care education to residents. Engaging with residents and family members in a compassionate and impactful manner is essential to create a lasting and positive experience. The position also requires fostering communication by regularly engaging with physicians, pertinent healthcare providers, and family members. Additionally, the role involves hiring, training, scheduling, and leading the clinical team to provide exceptional care experiences. Coordinating the Community's infection control program while maintaining state regulatory compliance and overseeing the Community's medication management program are also key responsibilities. The position contributes toward quality assurance and performance improvement initiatives and complies with all rules and regulations regarding confidentiality and privacy of resident information.
