Each year, HCAM assists over 80,000 individuals across the state, offering personalized guidance, health education, and advocacy to ensure every Marylander has an equal opportunity to achieve optimal health. healthcareaccessmaryland.org Job Summary: The Program Director serves as senior management for the MATCH program with specific focus on ensuring case management procedures meet the Baltimore City Department of Social Services (BCDSS) contract scope of work. In addition, the Program Director will ensure that best practices according to the American Academy of Pediatrics (AAP) and the Child Welfare League of America (CWLA) guide the process to ensure children's medical and behavioral health needs are met.

Requirements

  • Master’s degree in Nursing, Public Health, or Social Work or other graduate degree.
  • Five (5) years of related experience working in an appropriate field with at least two (2) years’ experience of overall responsibility for program direction, coordination, and implementation.
  • Years of experience may be substituted for education.

Responsibilities

  • Direct a case management staff of 60+ employees including Registered Nurses, Licensed Social Workers, Community Health Workers, and administrative personnel.
  • Lead and manage the implementation of evidence-based practices developed to address somatic and behavioral health needs of children/youth in foster care with Baltimore City Department of Social Services (BCDSS) to improve health outcomes
  • Create, review, and amend policies, procedures, and processes as needed in coordination with BCDSS leadership and HCAM leadership
  • Build and operationalize the program to advance the mission and vision of HCAM
  • Analyze and synthesize data to ensure the successful delivery of the program metrics to the funder.
  • Design strategic initiatives, implement program work plans and establish long term objectives to mitigate barriers to comprehensive health care and adversely affect the well-being of the children in foster care through BCDSS.
  • Develop and support strategic initiatives and community partnerships to remove actual and perceived barriers to health care access.
  • Examine and evaluate program effectiveness, and make changes as necessary in conjunction with Medicaid, HealthChoice, and Managed Care Organizations.
  • Provide financial oversight in conjunction with the Chief Finance Officer for the program’s contractual budget by analyzing and using cost benefit analysis for budgetary expenditures
  • Ensure that all expenditures are monitored, and reconciled; and program accounts are solvent
  • Create a cohesive work team that works collaboratively to achieve program goals through leadership and coaching
  • Oversee the recruitment, performance, and training of team members and continually review strengths and weaknesses of staff to maintain a well-trained, highly involved, and motivated team
  • Maintain compliance with all HCAM and BCDSS policies and regulations
  • Function as the primary contact for the MATCH Medical Director and Behavioral Health Consultant for BCDSS.
  • In collaboration with the MATCH Medical Director, formulate and provide oversight for the Continuous Quality Assurance process for all aspects of the MATCH Program
  • Develop, manage, and support the program’s partnerships and relationships within and outside of the MATCH program; including, but not limited to HCAM, BCDSS, community partners and healthcare providers
