About The Position

By The Bay Health, a non-profit established in 1975, set the standard for hospice in the U.S. by emphasizing the role of the patient in making important medical decisions. Our spectrum of home-based services now includes Skilled Home Health Care, Palliative Care, Adult Hospice Care and Pediatric Care. Our team approach strives to address practical, social, emotional and spiritual aspects of care, with the goal to maximize quality of life for our patients, caregivers and families. This full-time position offers competitive pay, generous benefits and a supportive work environment. The Director of Home Health directs and manages daily operations of Home Health Services. This includes, management of the annual department budget, staff productivity, and analysis and management of variances. This role keeps abreast of regulatory requirements and maintains state of survey readiness, directs patient satisfaction program and referral relations, including complaint management, and provides oversight of quality, staff education, and competencies Schedule: Monday-Friday; 8:30am-5:00pm Location: Larkspur Office; 4 days onsite required By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.

Requirements

Degree as a registered nurse. Bachelor’s degree in a health-related field; BSN preferred.
Minimum of three years supervisory experience in a health care (preferably Home Health) setting.
Current license as a registered professional nurse in the State of California

Nice To Haves

Home Care Manager Certification, Home Care Administrator Certification preferred.

Responsibilities

Coordinate and oversee all direct and indirect patient services provided by home health personnel.
Provide guidance and counseling to home health managers to assist them in continually improving all aspects of home health services provided through agency personnel.
Collaborates with colleagues to support patients’ transitions to other service lines.
Implement EPIC upgrades.
Maintains TJC accreditation.
Works with Quality Assurance on CAHPS scores
