About The Position

A high-volume, New York–licensed home care agency is seeking a senior-level Director of Coordination to oversee all care coordination operations. This role ensures uninterrupted patient coverage, efficient caregiver utilization, regulatory compliance, and consistent team performance across the organization.

Responsibilities

• Oversee all care coordination operations across the agency
• Manage Care Coordinators and Supervisors, enforcing accountability and performance standards
• Ensure full staffing of authorized cases and rapid response to callouts and emergencies
• Oversee live-in, split cases, overtime usage, and scheduling adjustments
• Collaborate closely with HR, Payroll, Recruitment, Nursing, Intake, Authorizations, and Retention
• Resolve escalations related to staffing, caregivers, and patient coverage
• Ensure accurate and timely documentation of coordination activities
• Track and report key performance metrics including unstaffed hours, overtime, response times, and complaints
