We are looking for a highly experienced Director of Nursing (DON) Registered Nurse to join our leadership team as Director of Nursing/Clinical Services of Home Health! Requirements

  • Graduate of an accredited school of Nursing.
  • Valid State license as an RN.
  • Three to Five (3-5) years of progressive management or supervisory experience in the adult Home Health setting.
  • Previous Director of Nursing or Director of Clinical Services experience in Home Health; preferably in an agency with 500+ patient census.
  • Proficient in the use of Electronic Medical Records - Experience in HCHB. (Home Care Home Base)

Responsibilities

  • Establish and direct the implementation of all written policies and procedures that reflect the goals, objectives and requirements of the Clinical department; ensure that all Clinical associates understand and follow established policies, procedures and systems.
  • Work with corporate clinical team in the development and implementation of departmental policies and procedures.
  • Communicate changes in the Nurse Practice Act to nursing personnel & violations of the Nurse Practice Act to the State Board of Nurse Examiners.
  • Maintain the confidentiality of all pertinent patient care information to ensure patient rights are protected.
  • Coordinate interdisciplinary patient care management efforts.
  • Ensure effective and adequate organization and staffing of the Clinical department.
  • Ensure that Clinical interventions meet the personal, physical and cognitive needs of each patient as well as maximize their self care capacities, identify, independence, choice and opportunity for social interaction.
  • Monitor the outcomes of clinical service activity by evaluating the performance of nursing staff, ensure compliance with all community and regulatory agency standards, policies and procedures, coordinating community committees and monitor the effective and efficient use of budgeted resources (associates, supplies, equipment).
  • Ensure that all applicable State and Federal employment laws and guidelines are followed by all clinical department associates.
  • Ensure that all clinical department associates are appropriately oriented, onboarded and trained according to company policy and procedure.
  • Develop and deliver training and in-service presentations.
  • Ensure associate and patient health and safety programs are implemented to provide a safe environment.
  • Actively maintain cohesive public and professional relationships with patients, families, physicians and the community.
  • Maintain ongoing education as it relates to continuing licensure and maintenance of professional skills.
  • Ensure adherence to established budgets for clinical department while also ensuring that quality of patient care is not affected.
  • Promptly investigate and properly documents any allegations of abuse or neglect.
  • Assure prompt notification to patient’s physician and responsible parties.
  • Interview candidates for employment in clinical department positions.
  • Review, discuss, evaluate and document clinical department associates’ performance.
  • Participate in the selection of patients to be admitted by evaluating the level and amount of care required by prospective patients in relation to existing clinical capabilities.
  • Ensure compliance with all federal, state and local regulations by conducting periodic self-surveys by outside agencies and correcting areas of non-compliance
  • Discuss/implement plans of action to correct deficient areas identified on quality reviews.
  • Oversight of all investigations regarding patients and clinical department associates.
  • Monitor and review patient incidents to identify possible legal issues and to identify patterns that may require preventive intervention.
  • Ensure that clinical staff is educated and utilizes infection control practices that protect staff, patients and families.
  • Ensure the resolution of complaints regarding the clinical department from patients, families or other responsible parties, medical staff volunteers and outside agencies affiliated with the community.
