About The Position

An established acute-care hospital in the Panama City region is seeking an experienced Director of Case Management to lead clinical resource management, care coordination, and discharge planning across a high-volume environment. This position oversees Case Management and Social Work teams, ensuring consistent alignment with regulatory requirements, organizational standards, and performance targets. This is a strategic, high-impact leadership role requiring strong clinical judgment, operational expertise, and the ability to partner effectively with executive and interdisciplinary teams.

Requirements

  • 3+ years of recent acute hospital Case Management experience (within the last year).
  • 2+ years of leadership experience in Case Management.
  • Bachelor’s degree (Nursing, Social Work, or related field).
  • Active licensure as RN, LCSW, or LMSW.

Nice To Haves

  • Master’s degree in Nursing, Health Administration, Business Administration, or a related field.
  • Experience overseeing both Case Management and Social Work teams.
  • Strong executive presence, communication, and presentation skills.

Responsibilities

  • Lead and oversee all operational aspects of Case Management, including staffing, workflow optimization, onboarding, competencies, and performance outcomes.
  • Ensure effective execution of core case management functions, including utilization management, discharge planning, and care coordination.
  • Maintain compliance with internal policies, regulatory standards, and documentation requirements.
  • Allocate departmental resources to support productivity, quality, and staffing needs.
  • Monitor department performance using data analytics while developing and implementing sustainable improvement strategies.
  • Collaborate with executive and clinical leaders to achieve financial, operational, and quality targets.
  • Build and maintain effective working relationships with physicians, nursing leadership, ancillary departments, compliance teams, and legal partners.
  • Drive team engagement, professional development, and accountability.
  • Serve as a subject matter expert in case management and provide guidance on process improvement initiatives.
  • Support organizational mission, values, and code of conduct.
