About The Position

We are seeking an experienced clinical leader to drive the operational success and strategic growth of our remote care team. The Director will oversee the performance of our remote clinical staff, manage high-stakes client relationships in the post-acute space, and lead outcomes-based initiatives to reduce hospital readmissions. This is a remote leadership role requiring a blend of clinical expertise, operational acumen, and the ability to translate complex data into actionable strategies for Executive Leadership and the Board.

Requirements

  • Licensure: Active clinical license required (LCSW, RN, LPN, OT, PT, or similar).
  • 5+ years of experience in healthcare leadership, specifically within Post-Acute Care, Skilled Nursing (SNF), Home Health, or Care Transitions.
  • Proven experience managing remote/distributed clinical teams is strictly required.
  • Strong proficiency in data analysis and Excel
  • Experience with EHR systems and Health Information Exchanges
  • Exceptional presentation skills

Responsibilities

  • Team Leadership & Remote Management
  • Lead and mentor a distributed, remote team of clinical professionals
  • Oversee comprehensive performance management cycles
  • Foster a culture of recognition and success
  • Manage staffing deliverables, workflow optimization, and training requirements for new and existing clinical staff.
  • Client Support
  • Serve as the primary care coordination escalation point and strategic partner for key post-acute and skilled nursing facility (SNF) partners
  • Develop content for and participate in client facing business reviews, demonstrating the value of care coordination
  • Collaborate with Client Operations and Clinical Operations to drive client satisfaction and retention.
  • Outcomes Development & Quality Improvement
  • Drive clinical strategy to reduce rehospitalization rates; lead the development of white papers and outcome studies quantifying post-discharge success.
  • Analyze clinical data to identify readmission trends and create targeted clinical interventions to mitigate
  • Oversee regulatory and compliance tasks
  • Executive Reporting & Operations
  • Prepare and present high-level operational updates for internal stakeholders
  • Develop and refine standard operating procedures (SOPs) for clinical workflows.
  • Collaborate with cross-functional leadership to align clinical capacity with business goals.
