About The Position

The purpose of this job is to lead our team in Care Coordination, which focuses on ensuring the most appropriate use of medical services and resources for our hospitalized patients.

Requirements

• Possess a Social Work degree.
• Must possess a valid New York State Social Work licensure.
• Must have experience in case management and/or utilization review in acute care, home care or insurance setting.

Nice To Haves

• Bachelor's preferred, master's degree a plus.
• Prior experience with Meditech E.H.R. information systems is a plus.
• Familiar with commonly accepted medical necessity guideline resources.
• Familiar with managed care industry UR program requirements.
