Establishes utilization review, care coordination and discharge planning policies and procedures in accordance with regulatory requirements, cost efficient principles and high quality nursing quality measures. Collaborates with internal and external partners along the continuum of care.

Requirements

  • Master’s degree in Nursing or related field.
  • Current Massachusetts RN license and registration.
  • Two years of previous management experience in hospital or healthcare environment.
  • Minimum five years’ experience in Case Management.
  • Knowledge of Utilization Review and Interqual Criteria.
  • Demonstrated ability to plan, organize and evaluate programs and to analyze fiscal data and develop departmental budgets.
  • Excellent written and oral communication skills.
  • Knowledgeable and skilled in the use of personal computer software.

Responsibilities

  • Designs work processes and assignments of care coordination staff including care management nurses, utilization review staff, and support staff in a manner that promotes care management performance that optimizes clinical and financial outcomes, and patient and staff satisfaction.
  • Identifies objectives and creates performance goals upon which to evaluate the performance of care coordination staff and of the program.
  • Integrates innovative technology into the care coordination process to improve programmatic effectiveness, enhance staff satisfaction and clinical outcomes for patients.
  • Promotes integration of clinical guidelines, performance improvement initiatives, and patient satisfaction efforts into the care coordination program.
  • Ensures that the care coordination functions are in compliance with all existing regulatory and reporting requirements and maintains awareness of changing requirements.
