Lexington Regional Health Center Title: Director of Care Coordination & Social Services Effective Date: March 2, 2026 Supervisor: Chief Operating Officer/Chief Nursing Officer Department: Nursing Service FLSA Status: Exempt Principal Duties and Responsibilities Provides leadership and oversight of transitional care, discharge planning, utilization review, swing bed coordination, chronic care management, and social services. Ensures patient-centered, interdisciplinary coordination of care across the continuum, addressing clinical, psychosocial, financial, and community resource needs while monitoring quality, safety, and regulatory compliance. Coordinates care and discharge planning of hospitalized patients through multidisciplinary rounds, huddles, and discharge planning meetings. Assesses and addresses patient discharge needs including clinical, financial, emotional, spiritual, and community resource needs. Manages and oversees swing bed services, utilization review, and chronic care management programs. Provides or coordinates patient and family education related to discharge planning and post-hospital care. Oversees quality, utilization, and performance data including readmissions, core measures, Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS), and population health metrics. Leads hospital committees including discharge planning, hospital acquired conditions and utilization review. Collaborates with nursing leadership, medical staff, and executive leadership to develop and evaluate programs and services. Develops and manages departmental budgets including staffing, operating costs, equipment, and supplies. Ensures compliance with regulatory standards, infection control practices, and patient safety initiatives. Provides direct supervision of care coordination, utilization review, social services, and related staff. Serves as liaison between the hospital and community agencies and resources. Participates in patient care activities that could include blood exposure and risk to bloodborne pathogens (eg: lacerations, -handling of blood-contaminated specimens, etc.). Responsible for ensuring the environment meets appropriate governing body standards. Ensure patients and visitors follow current infection control guidelines. Maints patient and staff safety through the use of patient safety tools (TeamSTEPPS, Just Culture, etc.). Serves on various committees as assigned. Regular attendance at the assigned work location is required. Performs all other duties as assigned.
