About The Position

The Director of Care at Home Clinical Services provides strategic and operational leadership for clinical service within the Care at Home portfolio, including Home Infusion Therapy, Remote Patient Monitoring (RPM), Mobile Vascular Access nursing, private duty nursing, hospice, etc. This role ensures high-quality, patient-centered care across the Care at Home continuum while driving performance, clinical outcomes, compliance, and alignment with health system strategic priorities.

Requirements

  • Bachelor's Degree in Nursing (BSN)
  • 5 years of clinical leadership experience
  • (RN) REGISTERED NURSE Licensed in the state of Texas

Nice To Haves

  • Master's Degree in Nursing, Health Administration, or related field preferred
  • Experience in post-acute/Care at Home services, infusion therapy, RPM and/or mobile nursing programs strongly preferred
  • (VA-BC) Vascular Access Board Cert or
  • Infusion certification

Responsibilities

  • Provide clinical leadership and oversight for in-home infusion services, RPM, and mobile vascular access programs.
  • Develop, implement, and evaluate clinical protocols, care models, and workflows to support safe, efficient, and high-quality care delivery.
  • Collaborate with interdisciplinary leadership (nursing, pharmacy, IT, operations, finance) to scale and optimize services across the health system.
  • Participate in long-term planning and development of strategic initiatives that enhance continuity of care and reduce hospital utilization.
  • Ensure appropriate triage, onboarding, and coordination of care for RPM patients, integrating biometric data monitoring with clinical decision support.
  • Supervise clinical staff including home infusion nurses, vascular access nurses, and other direct reports‿supporting competency development, licensure compliance, and performance evaluations.
  • Implement quality metrics, dashboards, and audit processes to monitor patient safety, infusion reaction protocols, line care outcomes, and RPM escalation responses.
  • Lead quality improvement initiatives to improve outcomes such as avoidable hospitalizations, timeliness of interventions, and patient satisfaction.
  • Ensure adherence to CMS Conditions of Participation, accreditation standards (ACHC, Joint Commission, or URAC), and organizational policies.
  • Serve as a clinical liaison between Care at Home services, inpatient units, case management, specialty physicians, and community partners.
  • Provide clinical input on technology integration (Epic, RPM platforms, supply chain, pumps/devices) to improve workflow and documentation.
  • Performs other duties as assigned.
