About The Position

The Director of CalAIM Pre-Release Services is responsible for the oversight, development, and implementation of CalAIM justice-involved pre-release services. This position ensures program compliance with state and managed care requirements, coordinates services for individuals transitioning from incarceration to the community and supervises staff responsible for Enhanced Care Management (ECM), care coordination, and reentry planning. The Director works closely with correctional facilities, managed care plans, community providers, and internal program leadership to ensure seamless care transitions and successful community reintegration for justice-involved individuals. This role provides leadership and oversight for CalAIM Justice-Involved Pre-Release services, ensuring effective implementation and operational integrity. It also ensures program compliance with CalAIM, Medi-Cal, and Managed Care Plan requirements, including Enhanced Care Management (ECM) enrollment and service coordination standards. The Director develops and implements policies, procedures, and workflows that support pre-release care coordination and transition planning, and oversees the identification, assessment, and enrollment of eligible incarcerated individuals into CalAIM services. The position directs the development of individualized care plans and ensures timely coordination of medical, behavioral health, housing, substance use treatment, and social service referrals prior to release. It establishes and maintains systems to support effective warm handoffs between correctional facilities and community-based providers, and serves as the primary liaison with Managed Care Plans, correctional institutions, and community partners participating in CalAIM initiatives. The Director supervises and supports program staff, including pre-release coordinators, ensuring high-quality service delivery and staff accountability. Responsibilities also include monitoring program performance, service outcomes, and operational metrics to ensure program effectiveness and contract compliance, and overseeing documentation, reporting, and data tracking requirements in accordance with regulatory and contractual standards. The Director ensures services are documented and delivered in accordance with CalAIM, Medi-Cal, and Managed Care Plan billing requirements to support accurate and compliant reimbursement, and leads quality improvement efforts to strengthen service delivery and improve participant outcomes. Finally, the Director represents the organization in county, state, and stakeholder meetings related to CalAIM justice-involved services and reentry initiatives, embraces and embodies the mission, vision, guiding principles, clinical vision, and goals of WestCare Foundation, and performs other duties as assigned.

Requirements

  • Ability to perform the duties and responsibilities satisfactorily.
  • Valid driver’s license and ability to obtain and maintain WestCare’s vehicle insurance.
  • Ability to obtain and maintain successful background clearance.
  • Comply with new requirements, including, but not limited to vaccinations that are issued by local, State, Federal, and/or Funders that WestCare may have to comply with.
  • CPR Certification.
  • First Aid Certification.
  • Annual Tuberculosis test.
  • Minimum 5 years of experience in healthcare, behavioral health, reentry services, or care coordination.
  • Minimum 3 years of supervisory or program management experience.
  • Bachelor’s degree in social work, public health, criminal justice, behavioral health, or related field.
  • Must complete required training related to CalAIM Justice-Involved services, documentation standards, and confidentiality requirements established by the California Department of Health Care Services and WestCare Foundation.

Nice To Haves

  • Experience working with justice-involved populations preferred.
  • Knowledge of Medi-Cal, CalAIM, and Enhanced Care Management programs preferred.
  • Master’s degree in social work, public health, criminal justice, behavioral health, or related field.
