The Director of Integrated Care Management provides direction and leadership in care management and coordinates the delivery of health care services. This leader reviews processes and determines performance improvement initiatives and develops and implements care management programs. This Director monitors expenditures related to care management programs and ensures compliance with all state and federal regulations related to care management. This leader collaborates with other leaders in order to ensure efficient and purposeful integration of services for the patient.

  • Ensure compliance with all state and federal regulations related to care management.
  • Collaborate with other leaders to ensure efficient integration of services.
  • Bachelor's Degree in Nursing required; Master's Degree in Nursing preferred.
  • Active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license required.
  • Licensed Social Worker (LCSW) preferred; must be obtained within 2 years of hire if applicable.
  • 7-10 years of integrated care management experience required.
  • Certification in Case Management preferred.
  • Strong knowledge of Case Management principles and standard practices required.
  • Knowledge of community resources and appropriate venues for care delivery required.
  • Knowledge of state and national regulatory agency guidelines required.
  • Strong clinical background with relevant leadership experience supporting patient care transitions required.
