The Director, Hospice role falls under the Senior Options advisory services offered by Westminster-Canterbury, focused on providing the solutions and tools your organization needs to bring Home and Community Based Services to your organization. We will also provide your organization with additional knowledge of your local health care market, and facilitating better account-targeting to obtain more significant market share. To learn more: https://senioroptions.net/ The Director, Hospice is responsible for all aspects of hospice including hiring, training and supervising the hospice team members and leading and facilitating the Hospice Interdisciplinary Team. The Director, Hospice is responsible for providing input to ensure the group is meeting or exceeding program budgeting. This leader promotes communication and collaboration in developing, monitoring, evaluating and revising hospice patient care plans. This leader also audits interdisciplinary documentation to assure accuracy and timeliness.

Requirements

  • Three years of experience in Hospice Care, with at least two of those years in a supervisory role.
  • Knowledgeable of applicable local, state, federal regulations.
  • Must have an unencumbered Virginia license as Registered Nurse.
  • Must maintain current CPR certification (Basic Life Support).
  • Strong computer skills, including Excel, Word, PowerPoint
  • Excellent verbal and written communication skills
  • Demonstrated ability to meet deadlines
  • Expected to be able to perform and deliver results work under limited supervision
  • Expected to be able to multitask, successfully handling various projects and schedules simultaneously
  • Must have experience with all types of office equipment; copier, fax, computers, multi-line phone system
  • Valid state driver’s license & satisfactory motor vehicle driving record
  • Proof of motor vehicle insurance

  • COVID-19 Vaccinated. As of now, the 2025–2026 COVID-19 vaccine formulation is being distributed to providers and pharmacies; however, it has not yet received official approval from the CDC. Responsibilities

  • Hire, train, supervise and monitor the performance and skills of Interdisciplinary Team members.
  • Serve as an accessible resource for clinicians in the field, providing guidance and assistance as needed whenever questions or situations arise.
  • Supervise the implementation of the patient’s plan of care, receives referrals, assigns cases, audits, organizes, oversees, integrates and modifies the resources necessary to reach desired outcomes.
  • Confer with the Medical Director regarding any questions about an individual’s eligibility for services.
  • Evaluates the effectiveness of the care plan on an ongoing basis.
  • Provide ongoing clinical assessment and provides care to clients and families.
  • Assure an accurate evaluation of client needs and develops and implements a comprehensive interdisciplinary care plan that addresses those needs.
  • Follow current professional standards of practice and agency approved protocols.
  • Audit and maintain current and accurate client records through use of laptop computers and EMR chart documentation in accordance with agency policies, federal, state, and local guidelines.
  • Assist the Interdisciplinary Team in establishing appropriate goals, priorities, and plans of care.
  • Completes clinical record reviews quarterly.
  • Participates in data collection, analysis and performance improvement projects.
  • Collaborate with other disciplines, recommends and/or processes internal or external referrals in order to meet the comprehensive needs of the clients.
  • Facilitate interdisciplinary team meetings with facilities and/or hospice personnel to promote coordination of care and discussion on interdisciplinary team involvement.
  • Manage the hospice budget
  • Supervise and manage employees, including completing employee evaluations and consulting with Human Resources and Administrator to take action regarding disciplinary and performance challenges.
  • Ensure organizational compliance with legal, regulatory and accreditation requirements.
  • Assist in managing conflict and complaint management/resolution
  • Ensure Hospice team stays current with clinical information and practices, including hospice Medicare benefit eligibility requirements.
  • Direct hospice team in performance of their duties including admission, discharge, transfer, revocation and provision of service to patients.
  • Ensure services provided by other agencies are authorized by hospice.
  • Ensure collaboration with agencies and vendors for effective management of services.
  • Ensure standards of ethical clinical practice are maintained.
  • Provide staff development including orientation, in-service education, continuing education and evaluation of staff.
  • Provide professional nursing service as needed.
  • Responsible for scheduling a similarly qualified alternate to be available in their absence.
  • All other duties as assigned.
