Position Purpose: The Director, Community Health & Well-Being (CHWB) works in collaboration with the Chief Medical Officer to advance the Health System’s ability to be a national leader in health equity and community impact by removing barriers to care, reducing health disparities, and improving outcomes for people experiencing poverty and other vulnerabilities. This position is also responsible for implementing strategies to promote the well-being of colleagues and leading, developing and overseeing all aspects of the CHWB and Health Equity and Human Impact (HEHI) strategy, with a focus on bridging internal and external relationships in communities throughout the regional service area. This position leads community benefit compliance and reporting, triennial community health needs assessment and implementation strategy and integrating social care programs to address patient health-related social needs to improve health. What you will do: Knows, understands, incorporates and demonstrates the Trinity Health Mission, Vision and Values in behaviors, practices and decisions. Participates in the development and implementation of a framework to connect clinical and social care across the region, including the development of a community health worker infrastructure. Advances partnerships to develop a seamless system of social care support with multiple partners including Health Ministry Hospital and Ambulatory leaders, government, academic institutions, payers, community-based organizations, and faith-based organizations. Develop and sustain strategic and authentic community partnerships that advance clinical and social care integration and improves community conditions through policy, system and environmental change strategies. Oversees the development and implementation of policies, procedures, tools and training across Health Ministry to ensure compliant, comprehensive and accurate reporting of community benefit activities as required by IRS regulations. Oversees analysis of the healthcare landscape to identify potential opportunities for advancement of community well-being, health equity and the Common Good inclusive of anchor strategies (local hiring, local sourcing, and local investing). Identify and advance internal policy and practice changes that improve health and well-being for people experiencing poverty and other vulnerabilities. Participates as the CHWB/HEHI representative on the Regional Integrated Clinical Leadership Team. Oversees CHWB department and programming across a region. Interviews, hires, orients, oversees training and development, schedules, assigns work to, evaluates the performance of and, when necessary, disciplines, all direct reports. Ensures compliance with established personnel policies when operating the Department. Supports the development and execution of enterprise strategies that advance health equity, inclusive workplace practices, and community wellbeing. Develops and implements aligned programs, policies, and partnerships tailored to local needs while contributing to system-wide goals. Tracks impact using defined metrics and adapts plans based on data and environmental shifts. In partnership with the Chief Medical Officer develop long and short-term goals, objectives, plans and budgets for all programs within CHWB to ensure continued quality of services through planning across the region with other functional areas. Serves as a lead convener of stakeholders, representing regional Ministry's commitments to advancing health equity, the Common Good community health improvement and partnerships. Lead sustainability and growth efforts through strategic fundraising, grant writing and billing for eligible services for CHWB programs and initiatives. Maintains a working knowledge of applicable Federal, State and local laws and regulations, the Trinity Health Integrity and Compliance Program and Code of Conduct, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical and professional behavior. Support the implementation of competency-building strategies and training programs that elevate understanding and accountability for inclusive leadership. Partners with internal learning teams to embed equity and cultural humility into leadership development pipelines.
