The Director, Community Health & Well-Being (CHWB) works in collaboration with the Chief Medical Officer to advance the Health System’s ability to be a national leader in health equity and community impact by removing barriers to care, reducing health disparities, and improving outcomes for people experiencing poverty and other vulnerabilities. This position is also responsible for implementing strategies to promote the well-being of colleagues and leading, developing and overseeing all aspects of the CHWB and Health Equity and Human Impact (HEHI) strategy, with a focus on bridging internal and external relationships in communities throughout the regional service area. This position leads community benefit compliance and reporting, triennial community health needs assessment and implementation strategy and integrating social care programs to address patient health-related social needs to improve health.
