About The Position

The role encompasses 24-hour responsibility and accountability for the effective and efficient operation of all aspects in the Case Management and Social Services departments. Responsible to work closely with internal and external parties to support optimum hospital resource utilization and transitions.

Requirements

• Associate Degree required, Bachelors’ Degree in nursing or related health care field preferred. Master’s Degree desirable.
• Previous management experience and minimum of 5 years’ experience in acute hospital nursing.
• Knowledge of Pathophysiology and standard treatment for DRGs.
• Knowledge of Healthcare delivery system components, understanding of acute/ non-acute care.
• Knowledge of common hospital and physician reimbursement methodologies and impact on UM/DC processes.
• Demonstrated managerial/ supervisory skills.
• Ability to develop processes to reach a goal.
• Current California RN license.
• A current American Heart Association (AHA) BLS card is required.
• Basic knowledge of computer programs, such as Microsoft Word, Power Point, and Excel.
• Must be able to perform essential physical job requirements.

Nice To Haves

• Preferred experience in Millimam Care Guidelines, discharge planning and case management.
• Certified Professional in Utilization Review (CPUR) certification preferred.
