The Director Care Coordination will plan, implement, and administer comprehensive service delivery in case management, progression of care, social work services, discharge planning, and transitions in care. This role promotes and establishes effective communication between Care Coordination and other Hospital departments and members of the medical staff. The Director will keep informed regarding current regulatory and professional trends, changes, and standards applicable to the scope of Care Coordination and make appropriate recommendations or changes in policies, procedures, and/or programs to conform to regulation changes and standards. Additionally, the Director will keep staff informed of Hospital Organizational goals, strategic plans and initiatives, finances, and Senior Management communication to ensure identification with the overall Hospital mission and goals. With Managers, the Director establishes effective lines of communication for the completion of objectives, oversight of areas of responsibility, and to ensure consistency for all Department staff. The Director will oversee the continuous assessment and improvement of the care coordination department and report pertinent information regarding patient problems, personnel problems, progress regarding projects and initiatives, and any unusual occurrences to the Vice President. This position also leads in the development and implementation of strategies to achieve clinical, financial, and quality outcomes, participates in all assigned committees and Hospital initiatives, and facilitates and leads workgroups and initiatives as requested. The Director serves as the administrative organizational liaison with post-hospital providers and community partners including, but not limited to Home Healthcare, Skilled Nursing and Acute Rehab facilities, Assisted Living Facilities, Hospice including Inpatient Hospice provider, LTACH, and Durable medical equipment companies, and medical transport companies, including primary critical care ambulance provider. Finally, the Director participates in community planning related to the interests of the Hospital and services for patients and families.
