About The Position

The Director of Care Coordination at Mercy General Hospital is responsible for overseeing and managing clinical services, ensuring effective care coordination, and improving patient transitions through the healthcare system. This role involves collaboration with various stakeholders, including medical staff and community healthcare providers, to enhance service delivery and meet regulatory requirements.

Requirements

• Bachelor's degree in nursing or equivalent experience required.
• Minimum of three (3) years of progressive supervisory or management experience in an acute hospital setting.
• Current Registered Nurse (RN) license required.
• Care Management (CM) certification preferred.

Responsibilities

• Direct and manage planning, organizing, and staffing activities related to clinical services.
• Oversee performance improvement initiatives, including length of stay (LOS) reduction.
• Report needs within the Medical Center and Dignity Health, as well as to government and regulatory bodies.
• Guide Care Coordination activities in accordance with medical center policies and standard practices.
• Consult and collaborate with managers, physicians, and community healthcare workers on care management issues.
• Work closely with departments and post-acute service providers to streamline patient transitions post-discharge.
