About The Position

The Adult Skills Center (TASC) provides innovative and proactive services that empower individuals with diverse intellectual, developmental, and mental health needs to achieve their highest level of Independence and realize their greatest potential. The Direct Support Specialist II is under the supervision of the Program Coordinator and Program Manager. Instructor is responsible for the planning, developing and implementing a routine program of client’s daily needs. Responsibilities will include monitoring client progress towards goals, client's paperwork; participation in all reports and case notes as required and assisting client in accessing all community resources. Responsibilities may include contact with Regional Center Service Coordinators and participation in client-oriented meetings and IPPs with maximum supervision.

Requirements

  • Must have a high school diploma (or GED)
  • Must have organizational skills, writing skills, oral communication
  • Self-motivated and a willingness to participate in independent and supported living service settings.
  • Must have clean DMV record and reliable transportation with current insurance coverage, if required to drive for position
  • Must be able to successfully complete First Aid/CPR
  • Must receive criminal record clearance through the DOJ
  • Maintain all certifications up to date
  • Ability to work effectively as a team member
  • Other qualifications may vary by client’s needs, and may include physical assistance, lifting, toileting, medication monitoring, etc.
  • CNA EXPERIENCE REQUIRED
  • Ability to drive company and/or personal vehicle for work, including transporting clients to and from work and/or appointments as needed, maintain valid driver’s license and car insurance, if required to drive for position.
  • Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA and other federal, state and local standards, including meeting qualitative and/or quantitative productivity standards.
  • Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards.

Responsibilities

  • Assist, Instruct, supervise and/or monitor the client to complete comparative shopping
  • Assist, Instruct, supervise and/or monitor the client to complete paying bills within a monthly budget
  • Assist, Instruct, supervise and/or monitor the client to complete problem solving
  • Assist, Instruct, supervise and/or monitor the client to complete socialization
  • Assist, Instruct, supervise and/or monitor the client to complete cooking
  • Assist, Instruct, supervise and/or monitor the client to complete reporting to SSI
  • Assist, Instruct, supervise and/or monitor the client to complete reporting to DPSS
  • Assist, Instruct, supervise and/or monitor the client to complete reporting to DMV
  • Assist, Instruct, supervise and/or monitor the client to complete reporting to the Regional Center
  • Assist, Instruct, supervise and/or monitor the client to complete reporting to work
  • Assist, Instruct, supervise and/or monitor the client to complete washing/drying laundry
  • Assist, Instruct, supervise and/or monitor the client to complete enrolling in classes
  • Assist, Instruct, supervise and/or monitor the client to complete travel training
  • Assist, Instruct, supervise and/or monitor the client to complete reporting special incidents
  • Assist, Instruct, supervise and/or monitor the client to complete household chores/maintenance
  • Provide support and/or physical assistance for those who require lifting
  • Provide support and/or physical assistance for those who require transferring
  • Provide support and/or physical assistance for those who require toileting
  • Provide support and/or physical assistance for those who require hygiene/bathing
  • Provide support and/or physical assistance for those who require medication monitoring
  • Provide support and/or physical assistance for those who require medical Appointments
  • Provide support and/or physical assistance for those who require understanding and complying with physician’s instructions and diagnoses
  • Work in higher intensive caseloads with clients who require extra support and assistance
  • Complete daily care notes in Therap (or current platform) by end of assigned shift
  • Accurately report all hours of work via TASC’s time tracking system
  • Meet with Program Director/Coordinator to write/compose semi-annual and annual reports to funding sources
  • Attend all mandatory ILS/SLS staff trainings and events
  • Also encouraged to attend all other staff trainings/meetings
  • Responsible for completing and presenting two trainings to the team within every 12-month period
  • Perform other duties as assigned
  • Perform other off-site duties as assigned
  • Participate in weekly meetings, trainings, etc. as assigned by the supervisor
