The scope of the role involves supervising a single program or unit within a program, typically supporting 4-8 individuals in residential settings or 10-15 individuals in periodic, vocational or day programs. Duties are split between the provision of direct support, professional or program activities and supervision. Generally, the supervisor works 8 to 16 flexible hours per week in a supervisory capacity and provides direct support for the remaining hours. The role requires performing essential job functions related to services and supports, health care, stakeholder relations, personnel management, financial management, census management, maintenance, and other duties as required, including backfilling roles under supervision.

Requirements

  • High School diploma or equivalent
  • One-year related work experience
  • Must be 18 years or older
  • Working knowledge of computers
  • Current driver's license, car registration and auto insurance if driving on the behalf of the Company or providing transportation to individuals receiving services
  • Other licensure(s) or certification(s) where required by regulatory authority

Responsibilities

  • Supervise Direct Support Professionals
  • Oversee the day-to-day operation of the program including delivery of supports, implementation of person-centered plans, health care, advocacy, customer engagement, regulatory compliance and when applicable, oversight of the physical environment
  • Generally works 8 to 16 flexible hours per week in a supervisory capacity; and provides direct support for the remaining hours
  • Work regularly scheduled shifts as a direct support professional
  • Document as required; review individual records, billing and other program documentation routinely for accuracy and completion on a daily basis; maintain records
  • Assist the Program Director in development of individual program plans; train and monitor implementation of plans each day across all shifts
  • Attend and assist with routine progress meetings
  • Assist the Program Director in maintaining licensing compliance for program services and supports; implement Sevita compliance plan; manage individual rights implementation
  • Assure safety of individual’s funds and property when assigned; monitor spending and reconcile individual’s financial accounts monthly
  • Complete or assist DSPs in documenting incident reports; submit initial incident reports to the Program Director
  • Compile or accumulate census or attendance information; review data and attest to accuracy; submit accurate data to the Program Director or appropriate billing authority
  • Monitor health of persons being served; document; communicate with medical professionals as appropriate; implement preventive strategies to limit illness and serious incidents; review and monitor documentation across all shifts
  • Schedule or assist in scheduling health care appointments when applicable; may accompany individuals to appointments; may assist with communication; document visits; oversee follow through of orders; communicate results as appropriate with families, guardians, case managers, day programs or others
  • Accurately administer or support self-administration of medication and document delivery of medications and treatments when applicable; review all other medication documentation for accuracy and completeness, daily or weekly as required; review medication errors with employees; communicate medication changes and provide training across all shifts; maintain appropriate security of medications and supplies
  • Purchase and maintain medical supplies and equipment in consultation with Program Director; communicate with employees and may assist with training on proper use
  • Develop menus based on diets, preferences, and occasions, and with the exception of some in-home services, purchase or oversee purchasing of food and supplies in accordance with planned menus and individual choice or need; direct and monitor food service, consistent with individual plans
  • Maintain productive and professional relations with individuals, families, case managers and other stakeholders
  • Implement Sevita Customer Service Standards; train DSPs in delivery
  • Assist with recruitment as needed; interview and hire direct support professionals in consultation with the Area Director and Talent Acquisitions teams
  • Populate and maintain schedule to assure that all shifts are covered, and overtime is minimized
  • Assist with onboarding and deliver orientation and ongoing training concerning the individuals served, program plans, policies, procedures and physical environment; Verify training records are accessible in the home and are completed by all staff on the schedule
  • Prepare and conduct annual evaluations for Direct Support Professionals, in consultation with the Program Director
  • Coach employees as needed, correcting or directing employee behavior where required; may remove employees in the event of emergency; may recommend and participate in decisions regarding termination; consult with the Program Director on all matters of discipline
  • Responsible for employee safety and workers’ compensation within the program; implement and monitor compliance with safety standards
  • Assist with or lead monthly program meetings for direct support professionals; document monthly meeting agenda and attendance
  • Purchase food and household supplies when applicable; purchase office supplies and minor equipment; review and attest company credit card statements and monthly invoices; direct employees as necessary; monitor and attest to cost effective, accurate and applicable spending
  • Secure and safeguard individuals' served funds locally; use forms for money/debit card transfers; update financial ledgers weekly and maintain itemized receipts for purchases on behalf of individuals served
  • Review census, attendance information or other required documentation to assure accurate billing; monitor utilization between authorization and provision of services and supports
  • Monitor staffing hours for budgetary compliance
  • Capture daily census within systems; understand importance of accurate and timely service capture along with required documentation
  • Develop basic understanding of census, including how census is measured and census performance for locations; Escalate potential service disruptions
  • Support location preparations required for expected placements (e.g., room & furniture set-up, safety requirements, etc.)
  • Test drive vehicle monthly when applicable; inspect vehicle lifts and seatbelts monthly; report safety concerns immediately to supervisor or maintenance personnel; assure vehicle is washed and cleaned; train staff in safe vehicle operation and safely transporting individuals; ensure routine maintenance is performed
  • Schedule and monitor daily and seasonal housekeeping when applicable; oversee lawn and yard care
  • Maintain neat, clean and safe environment for individuals served
  • Make or arrange for minor repairs when applicable; report to and schedule repairs by maintenance personnel as required; monitor environmental safety; monitor water temperature, refrigerator and freezer temperature, and safety alarms and extinguishers on a monthly basis; replace furnace filters monthly
  • Monitor wheelchairs, lifts and other safety equipment; see that repairs are made promptly and correctly in the event of faulty equipment
  • Perform other duties and activities as required, including backfilling roles under your supervision
