About The Position

This Direct Support Professional is responsible for implementing individualized plans, attending to personal care needs, overseeing the health, safety, and well-being of the people supported, keeping nurses advised, participating in agency activities, transporting and accompanying individuals on community activities, assisting in daily facility-related duties, and maintaining documentation. Upstate Caring Partners is seeking energetic and motivated staff to work with individuals with disabilities to help them achieve their goals. The DSP provides care, support, and assists residents with activities of daily living, and serves as their advocate during appointments and community activities. Requirements

Valid NYS Driver’s License required
No previous experience needed

Responsibilities

Implement each individualized plan
Attend to personal care needs
Oversee the health, safety and well-being of the people we support
Keep nurses advised
Participate in agency activities
Transport and accompany people we support on activities in the community
Assist in daily facility-related duties
Maintain documentation
Provide care, support, and assist residents with activities of daily living
