About The Position
Provide direct support to program individuals in the Supported Living Program
Requirements
- Must be at least 18 years of age, possess a high school diploma or GED, OR have 18 months of verifiable work experience in providing support to individuals with developmental disabilities or elderly.
- A valid driver’s license, vehicle in operating condition and vehicle insurance is also required.
Responsibilities
- Assist program individuals with implementation of comprehensive plans of care
- Assist and/or train individuals in performance of activities of daily living
- Personal hygiene
- Light housekeeping
- Shopping etc.
- Empower individuals to develop leisure, social and employment skills
- Follow all guidelines & procedures related to EZ Labor (clocking in & out, reviewing time cards, requesting edit sheets, etc.).
- Monitor and report program individual’s compliance with all physician orders (including
- Medical and psychiatric)
- Maintain required documentation (progress notes, incident reports, etc.)
- Submit all required documentation timely and with all required signatures
- Report any significant concerns to supervisor
- Maintain training requirements
- Attend and participate in staff meetings
- Projects and duties as assigned
