Direct Support Professional -Personal Care Assistance: Assist individuals with daily activities, including personal hygiene, grooming, dressing, eating, and mobility. -Encourage Independence: Support individuals in developing independent living skills such as meal preparation, household chores, and money management. -Goal Implementation: Help individuals achieve their personal goals as outlined in their Individual Service Plan (ISP) by providing guidance, encouragement, and positive reinforcement. -Social and Recreational Support: Facilitate and encourage social interactions, leisure activities, and recreational outings to promote community engagement and a healthy social life. -Health & Safety: Ensure the well-being and safety of individuals by monitoring health conditions, administering medications as needed, and assisting with medical appointments. -Behavioral Support: Utilize positive behavioral interventions and strategies to support individuals in managing challenging behaviors and promoting positive actions. -Documentation: Maintain accurate and up-to-date records of services provided, including daily logs, progress notes, goals reporting, medication logs, finance ledgers, and incident reports as required. -Collaboration: Work as part of a team to support individuals and communicate with families, caregivers, and other professionals to ensure consistent care and services. -Emergency Response: Act promptly and effectively in emergency situations, ensuring the health and safety of individuals at all times.
