We are seeking a dedicated and compassionate Community Habilitation Worker to join our team. As a Community Habilitation Worker, you will provide essential support and assistance to individuals with special needs. Your role will involve working directly with clients and their families to ensure their well-being and promote their independence. Responsibilities: - Provide direct support and care to individuals with special needs, including assisting with daily activities and personal care - Implement behavior management strategies to promote positive behaviors and reduce challenging behaviors - Collect data on client progress and behavior using specified methods and tools - Collaborate with clients Circle of Support to develop and implement individualized care plans - Assist clients in developing essential life skills, such as communication, socialization, and self-care - Provide transportation for clients to appointments, activities, or community outings - Create a safe and supportive environment for clients, ensuring their physical and emotional well-being - Maintain accurate documentation of client interactions, progress, and any incidents that occur Skills: - Experience or knowledge in special education or caregiving is highly preferred - Proficiency in data collection methods and techniques - Valid driver's license and reliable transportation for client transportation - Knowledge of behavioral health principles and strategies - Experience in direct support or childcare roles is beneficial - Ability to work effectively with individuals with special needs
