Summary: As a community Habilitation DSP you are matched to an individual based on likes, location and availability. Generally you will be going out into the community with the individual. This position requires a great amount of commitment, self-motivation and solid communication skills. Essential Duties and Responsibilities include the following (Other duties may be assigned): The work is usually one to one Participates in a person centered approach to program plan development and participates in activities to build competencies and integrate individuals into their communities. Assists individuals we serve to identify social, recreation and other community resources the individual may wish to access and devise plans to achieve that access. Negotiates schedules and daily activities directly with the individual and/or caregiver. Establish and maintain regular hours with individual(s) that you work with Completes all mandated reporting requirements by Wildwood and regulating agencies.

Requirements

  • High school diploma or general education degree (GED).
  • A valid NYS driver's license and ability to drive and driving record acceptable for agency insurance coverage are required.
  • Access to a vehicle needed to provide services.
