About The Position

Role: To provide quality living and community support and be responsive to individuals needs in residential, work or community settings. Teach, support and facilitate choices for individuals in their homes, work or community settings.

Requirements

  • Prompt, consistent attendance is an essential function of this position.

  • Adhere to scheduled hours.
  • Adhere to individuals Person Centered Support Plans.
  • Meet agency standards of necessary supports to ensure safety, happiness, and well-being of individuals served.
  • Must be able to communicate effectively with all levels of personnel and individuals served.
  • Must demonstrate compassion for individuals with disabilities.
  • Basic mathematical, grammar and computer skills preferred.
  • Valid drivers license with a clean driving record.
  • Must be able to lift, push and/or pull a minimum of 50 pounds, stand for periods of time, bend, push wheelchairs, and provide physical intervention on a regular basis.
  • Due to the potential intimate nature of care and physical proximity involved with some of those receiving HCBS funded I/DD services through Disability Supports, and in order to promote a safe work environment for both the caregiver and those for whom we care, you may, from time-to-time, be required to wear PPE, (Personal Protective Equipment). If PPE is required, Disability Supports will provide it.

Nice To Haves

  • Prior direct care experience preferred, but not requested.
  • High School diploma/equivalent preferred.

Responsibilities

  • Implement Person Centered Support Plans for assigned individuals.
  • Assist with daily living skills including, but not limited to, meal preparation, general hygiene, toileting, general housekeeping, work and leisure activities.
  • Administer medication and treatment(s).
  • Complete documentation as required through the general course of duties.
  • Report to the appropriate Program Coordinator, Program Manager, Case Manager or Wellness Liaison any occurrences that affect the immediate well-being of the individual.
  • Attend all meetings as assigned and required.
  • If approved as a driver of agency vehicles, employee may transport individuals.
  • Other duties as assigned.
