Direct Support Professional- Supported Family Living
About The Position

Hands of Heartland is looking for compassionate and caring individuals to work with the people we support as a Supported Family Living Direct Support Professional (DSP). As a Supported Family Living DSP, you will work directly with people with developmental disabilities assisting them with day to day living activities. You will support individuals with various activities in a home environment and in community settings including meal planning, grocery shopping, or household chores.
Requirements
- Must be 18 years old.
- Must have a valid driver’s license and car insurance.
- Must be able to pass all background checks.
- Experience not required.
- Ability to work independently in a fast paced, team-oriented environment.

Responsibilities
- Assist with activities of daily living (ADLs) such as hygiene, dressing, mobility, and meal preparation, based on the individual’s level of need.
- Support the individual in developing and maintaining skills for independent living, which could include money management, household chores, self-advocacy, and communication. Based on individual's needs, goals, and support plan.
- Implement goals and objectives from the individual’s person-centered support plan and document progress as required.
- Monitor and promote the individual’s health and well-being while following safety protocols.
- Encourage social interaction and community inclusion through meaningful activities and relationship building.
- Maintain respectful, professional, and confidential communication with the individual, their family, and the interdisciplinary support team.
- Comply with all applicable policies, procedures, and state regulations related to waiver services.
- This position may require you to transport individuals in company vehicles. You will be paid extra for driving.
