Personal traits we are looking for: Dependable Flexible Fun Awesome communication skills The work of a Direct Support Worker is directed by a plan of care developed by the Oregon Department of Human Services. The direct support worker follows the individual service plan for each individual. Here are our current clients looking for additional support: Client 1: Female | 22yrs old | 97212 | Schedule: Mon, Wed, Thur (3-4 hr a shift) Likes: Must be close to close to clients age and into the same things, gym, dancing, music, getting out and about Needs: Energetic with dashes of enthusiasm, comfortable in a kitchen to help client, be driven to help client find motivation to get out and about, be self driven to help client succeed Client 2: Female | 5yrs old | 97230 | Schedule: 3-5 days a week 20-25hrs a week- 4pm-9pm (ma vary with summer hours) Likes: Playtime and being interacted with Needs: Someone that will work on communication skills, has experience with ADD and or sign language (is a must), experience with delayed speech, must be dog friendly Client 3: Female | 22yrs old | 97220 | Schedule: Tue/Thu/Fri Pick up from Job Corp 3:45pm- and keep her company for several hours Likes: loves learning, sweet, tenacious, great communication skills Needs: Help with individualized assistance in social and physical activities, ensuring she remains engaged, help her work towards her independence, help her engage in the community, Client 4: Female | 21yrs old | 97212 | Schedule: Tue- Fri- Mid afternoons until 7-8ish Likes: Songs, to talk about what she likes Needs: Prefers someone her own age and sex, needs support with cuing for restrooms and support Client 5: Male | 5 yrs old | 97212 | Schedule: Wed-Thur 3:45pm-8:30pm (schedule will vary in the summer), Sun 9am-8:30pm Likes: Loves to smile, be active and move around a lot, loves family and friends. and people in general, loves to learn Needs: Someone who is going to be engaging, and active with client, and show an interest in things client likes
