About The Position

Responsible for the direct care of adults in a program setting which supports the interests and needs of individuals, often with disabilities, including intellectual disabilities. Advances Delta’s mission and values.

Requirements

  • Ability to read, write and speak English; perform arithmetical calculations
  • Ability to use excellent oral and written communications skills.
  • Ability to use telephone, voice mail, copier, facsimile machine, computer and calculator.
  • Ability work effectively with people, facilitating their growth and development.
  • Ability to use acquired knowledge of supervisory and administrative principals and methods.
  • Ability to operate vehicles with a valid driver’s license in accordance with applicable state laws, insurance provider requirements and company policies and procedures.
  • Ability to assess information from plan information to facilitate interactions.
  • The ability to frequently exert up to fifty (50) pounds of force (lifting).
  • A high school diploma or equivalent (GED).
  • Exceptional interpersonal communication skills.
  • Excellent verbal and written communications skills.
  • Ability to work on assigned tasks as well as to accept direction on a given assignments.
  • Ability to resolve non-routine situations properly.
  • Ability to manage multiple and sometimes confliction priorities.

  • Ensure the implementation of high quality individual support plans.
  • Promote and follow Delta’s Risk Management Program for the health and safety of individuals and employees.
  • Engage individuals in meaningful activities at home and community
  • Assist in basic living activities of each individual including appearance, nutrition, recreation and socialization.
  • Pursue community integration for each individual
  • Act as a role model to the individuals in areas such as appearance, emotional stability and social behavior.
  • Encourage and work with individuals to progress towards their outcomes and record data
  • Contribute as a member of the interdisciplinary process in:
  • Developing objectives and outcomes
  • Evaluating progress toward independence in outcomes
  • Encouraging person-centered services
  • Complete and submit in a timely manner, required records and forms such as:
  • Incident/Unusual Incident Report
  • Case Notes
  • Communicate continuously with your Supervisor via email, text, phone, and other means of communication
  • Regular attendance is essential
  • Report unsafe conditions IMMEDIATELY
  • Participate in activities to protect the safety of employees and individuals of Delta
  • Adhere to the use of universal safety precautions and health precautions and health regulations with all individuals
  • Participate in training and education on health and safety issues and techniques yearly in a timely manner
  • Must be an approved driver and must maintain that status.
  • Perform other duties as directed to support service to the individuals, the division and Delta.
  • Perform such other duties that may be assigned for providing excellent customer service and for the most effective operation of the company.
  • Attend monthly staff meetings and all required training programs as assigned by the supervisor.
  • Transport individuals in personal and company vehicles as required.
  • Cooperate with company and regulatory agency personnel during any inspection or investigation.
  • Comply with Delta’s policies and DDD waiver standards.
