Requirements
- High school diploma or GED
- Valid driver's license and reliable transportation
- Ability to lift 25-50 lbs. (with assistance or lifting devices)
- Physical ability to bend, squat, push/pull, and kneel as needed
- A compassionate and patient mindset
Responsibilities
- Follow individual care plans and report any concerns to nursing or management.
- Administer medications and treatments with proper documentation, per policy.
- Support daily living needs, including: Personal care, Meal preparation and dietary support, Household tasks and errands, Health appointments and community integration, Building natural supports and social relationships.
- Promote health, safety, and a clean home environment.
- Remain calm and effective during challenging situations.
- Foster a positive, respectful team dynamic with coworkers, individuals, and their families.
- Accurately document individual progress and changes.
- Attend all required trainings, meetings, and ongoing professional development.
