The salary for this position : Minimum Salary:$19.00/hr, Maximum Salary: $19.00/hr - Overtime is eligible. This opportunity is available at our program located at 146th Ave in Springfield Gardens, NY
Requirements
- HS Diploma or GED required
- Valid NYS Driver’s license and must be able to pass insurance criteria. (offer may be extended with the contingency to obtain within one year)

Responsibilities
- Provide habilitation services to each individual as prescribed in their individual service plans (CFA, Individual Service plan, Habilitation plan)
- Assist each individual as needed, in skill development in areas such as activities of daily living, domestic living, recreation and socialization
- With appropriate training and supervision, administer medication to individuals as assigned and in compliance with management, medical and nursing policies and procedures
- Ensure assigned individuals ’nutritional needs are met according to prescribed diet, and assist with feeding as necessary
- Report immediately any observed/known deficiencies or own inability in providing habilitation services to clients, as well as all types of abuse and other facility problems, including physical plant/safety issues to management
