About The Position

Responsible for the security, safety, and wellbeing of the individuals attending the Day Habilitation program according to his/her specific needs and interests, to increase the quality of life for each person by promoting their respective level of independence.

Requirements

  • Physical ability to assist individuals as needed, which may include lifting from wheelchairs, bathtubs, beds, etc.
  • Demonstrated ability to interact in a respectful, dignified manner with individuals.
  • Ability to verbally communicate effectively with individuals, staff, and outside agencies to maintain the efficiency operation of the agency business.
  • Demonstrated written skills to adequately perform all required documentation.
  • Ability to follow verbal and written directions of supervisors, program plans, behavior plans, medical/medication instructions, etc.
  • Ability to complete and maintain all training required by licensing or the agency.
  • Demonstrated ability/understanding of basic life safety topics relevant to individuals' well-being (CPR, Danielle’s Law, and first aid skills, fire safety, etc.)
  • High School Diploma or GED.
  • Current, valid driver's license with acceptable driver's record from DMV.

Responsibilities

  • Implement goals, training, and community inclusion activities as per the individual's Day Habilitation Plan.
  • Work with individuals in groups or individually as assigned.
  • Document individuals’ progress daily, or as required.
  • Transport individuals as necessary to programs, activities, appointments, recreational functions, etc. as scheduled or requested by supervisors.
  • Ensure wheelchair-bound individuals are safely and securely strapped and unstrapped in agency vehicles when traveling.
  • Assist individuals in preparing and eating meals.
  • Assist individuals with personal grooming and hygiene (e.g., changing diapers) as needed.
  • Promote socialization and group discussions. Assist the individuals in planning collectively agreed-upon recreational/socialization activities.
  • Promote consumer progress through manipulative fine motor therapy, hygiene, positive self-image, and appropriate demeanor.
  • Other duties as requested by supervisors or agency administration.
