Requirements

  • High School diploma or equivalency diploma required.
  • A valid New York State Driver's License with 2 years of driving experience is required.
  • Experience working with adults with intellectual/developmental or other behavioral health disabilities preferred.

Responsibilities

  • Ensure that all activities enhance and support each person's social responsibility, independence, dignity and continued growth.
  • Implement goals designed to maximize each person served with a level of independence, including budgeting, grooming, nutritional planning, use of public transportation, and use of community resources.
  • Safely operate agency vans and/or vehicles as per the agency driving policy.
  • Complete and perform all techniques required in AMAP, SCIP, CPR and First Aid.
  • Coordinate evacuation and ensure safety in actual emergencies and fire drills.
  • Administer medication and/or monitor self-administration of medication as a certified AMAP.
  • Responsible for crisis intervention and incident reporting procedures.
  • Monitor and record progress on daily checklists, behavior contracts, quarterly summaries, and other documentation.
  • Communicate relevant information to other staff and supervisors, verbally and in writing.
  • Meet routinely with people served and the management team to review goal progress and update program plans.
  • Assist individuals in planning and following through on recreational activities.
  • Provide training in ADL and perform these functions for people served requiring total support.
  • Attend all scheduled staff meetings, mandatory in-service training sessions, and assigned annual meetings.
  • Act as a role model for people served in various settings.
  • Follow up on any aftercare/recommendations for treatment.
  • Maintain confidentiality at all times.
  • Perform other duties as requested.
