Our mission is to advocate for and provide support and services to people with developmental and other disabilities, emphasizing choice and full community engagement. SUMMARY: The Direct Support Professional provides self-care training and assistance in daily living to individuals with developmental disabilities residing in a group home setting.
Requirements
- High School or GED
- acceptable New York State Drivers License and 12 months driving history.
Responsibilities
- Must stay awake and not place themselves in a compromising position/place that is conducive to sleep
- Continuously assess the status of the physical building and the individuals throughout the duration of the shift.
- Provides individualized care as outlined in the individuals Life Plan, or ISP.
- Supports the residents to ensure their safety and well being.
- Observes, documents and communicates pertinent information such as behaviors, nursing, nutrition, clinical issues, etc., to facilitate assessment and development of treatment goals.
