About The Position

The Direct Support Professional III - LPN is a Licensed practical nurse who is a direct service position providing individual medical support and education to individuals in assigned programs advocating for all medical needs of assigned program participants focusing on optimal health for all individuals. Hours assigned by supervisor; on call responsibilities. With advanced qualifications or significant experience, this role will provide specialized or high-level support, while maintaining quality, service excellence and ethical standards. Direct Support Professionals are provided a paid New Employee Orientation and receive training on CPR/First Aid, Safety Care, Defensive Driving, and Medication Administration Program (MAP). They also receive program-specific training at the work site.

Requirements

  • High school diploma, GED, or HiSET required.
  • Licensed practical nurse required (LPN)
  • 1-3 years of related experience preferred.
  • CPR/First Aid and MAP certification required (paid training provided if needed).
  • Strong communication and organizational abilities.
  • Compassionate and patient approach to care.
  • Commitment to fostering inclusion and independence.
  • Ability to perform physical tasks, including lifting up to 50 pounds (or 200 pounds with assistance), and remain alert during shifts.

Responsibilities

  • Assess and coordinate healthcare services for individuals, ensuring timely follow-up and communication with relevant clinical and residential staff
  • Accompany and assist individuals with medical appointments in accordance with DDS guidelines
  • Document all nursing interventions, health updates, and medication changes in Electronic Medical Records (EMR) and communication logs
  • Monitor and maintain current medication and treatment orders, report side effects or adverse reactions as appropriate
  • Review and support implementation of Individual Service Plans (ISPs), including self-medication, behavior plans, hygiene, and therapy programs
  • Train residential staff on medication administration and health protocols
  • Ensure up-to-date and compliant health records in line with DDS standards
  • Communicate medical recommendations clearly to all program staff involved in the individual's care
  • Maintain safety and monitor for health-related risks within the program
  • Participate in staff meetings, mandatory trainings, and clinical supervision with the Health Care Director or program RN
  • Perform medical interventions and additional responsibilities as assigned
