About The Position

Momentum is hiring a Direct Support Professional II (DSP II) to provide support to adults with disabilities. As a Direct Support Professional II (DSP II), you will be responsible for the implementation of Individual Service Plans (ISP) that will lead to or continue to promote independence and personal skills, well assisting in providing a safe and secure home environment for persons supported in Momentum’s Independent and Community Living Services program.

Requirements

  • Must be 18 years of age or older
  • Current and valid California driver license, liability insurance, and reliable means of transportation; required
  • High school diploma or equivalent; required
  • Must have reliable phone/access to phone (monthly stipend)
  • Must be able to read, write, comprehend, and communicate effectively in English
  • Successfully pass all post-offer, pre-employment screening requirements, including criminal background check, drug screening, and provide proof of eligibility to work in the United States
  • Ability to run, push, pull, bend, squat, kneel, lift up to 50 lbs., and transfer individuals
  • Must be available to travel throughout Los Angeles and Orange Counties, as needed (mileage reimbursed)

Nice To Haves

  • Minimum of one (1) year experience working with people with developmental and/or other disabilities; preferred
  • Use of personal vehicle for assignments throughout service areas not normally assigned; highly preferred

Responsibilities

  • Provides individualized coaching in daily living skills
  • Assists with daily needs/tasks within persons supported home
  • Provides education and support in the use of community resources
  • Maintains accurate and current records in individuals’ communication logs regarding information relevant to Individual Service Plans (ISP)
  • Supports and assists persons supported in following their daily activity schedule including household meal preparation, maintaining sanitary and orderly conditions in their home, other household tasks, budgeting, ledgers, and transportation, as necessary
  • Prompts and assists persons supported in medication management
  • Teaches adaptive living skills and monitors the extent to which they are followed
  • Assists participants in all aspects of personal care, including hygiene and medical/dental appointments
  • Assists individuals in attending social activities to further social skills
  • Trains program participants to access public transportation and assist persons supported in accessing benefits such as Social Security, SSI, IHSS and Section 8 housing
  • Assists persons supported in weight bearing and non-weight bearing transfers
