Requirements

  • High school diploma or Equivalent required.
  • Valid NYS Driver License with at least 1 year of driving experience.
  • Ability to operate agency vehicles.
  • Capability to lift up to 50 lbs.
  • Evening and weekend shifts are preferred.
  • Must possess and maintain a personal cell phone for work-related communication; this is a condition of employment.

Nice To Haves

  • Previous experience as a Caregiver, Direct Care Worker, Nursing Assistant, Home Health Aide, or Companion is advantageous.
  • Preferred experience working with individuals with intellectual and developmental disabilities.

Responsibilities

  • Support individuals to reach their personal goals, from daily tasks to personal achievements.
  • Provide transportation to fun activities, medical appointments, and more, ensuring individuals stay connected with their community.
  • Support behavioral modifications and communication improvements, helping individuals thrive in every aspect of their lives.
  • Work with the individuals and other members of the team to develop service plans that aid the person in achieving their life goals.
  • Support and promote the individual’s health and safety.
  • Maintain accurate and thorough documentation of service activities, including daily progress notes, interactions, and any additional information as required by agency guidelines.
  • Assist with personal care, social interactions, meal prep, grocery shopping, and light housekeeping.
