Requirements

High school diploma or Equivalent required.
Valid NYS Driver License with at least 1 year of driving experience.
Ability to operate agency vehicles.
Capability to lift up to 50 lbs.
Evening and weekend shifts are preferred.
Must possess and maintain a personal cell phone for work-related communication; this is a condition of employment.

Nice To Haves

Previous experience as a Caregiver, Direct Care Worker, Nursing Assistant, Home Health Aide, or Companion is advantageous.
Preferred experience working with individuals with intellectual and developmental disabilities.

Responsibilities

Support individuals to reach their personal goals, from daily tasks to personal achievements.
Provide transportation to fun activities, medical appointments, and more, ensuring individuals stay connected with their community.
Support behavioral modifications and communication improvements, helping individuals thrive in every aspect of their lives.
Work with the individuals and other members of the team to develop service plans that aid the person in achieving their life goals.
Support and promote the individual’s health and safety.
Maintain accurate and thorough documentation of service activities, including daily progress notes, interactions, and any additional information as required by agency guidelines.
Assist with personal care, social interactions, meal prep, grocery shopping, and light housekeeping.
