About The Position

Requirements

• Must be 18 years of age or older
• Must have a valid driver’s license
• Ability to work in a group home, home-like setting
• Ability to communicate (verbally and written) with all levels of personnel, internal and external
• Staff are required to have 3 years experience working with individuals with severe mental illness, crisis, physical interventions, and behavior supports
• Staff must be well trained and have at least 3 years experience in using positive redirection and behavior modification programs

Responsibilities

• Performing personal care tasks, including feeding, ambulation, and medical monitoring
• Assist with fostering positive relationships between individuals served and their housemates
• Ensuring client safety and maintaining a safe environment
• Encouraging self-help activities
• Accompanying clients to scheduled appointments
• Implement formal/informal crisis plans, program plans and/or behavioral objectives using person-centered planning
• Maintain documentation, record keeping and participate in regular meetings and complete required training
• Take initiative when responding to a crisis, monitor the safety of person's served, and provide support and supervision in a positive therapeutic manner with the least restrictive methods of positive intervention
• Enable person's served to make choices in all areas of their lives
• Encourage use of public resources and recreational facilities; provide community integration opportunities
• Provide transportation for appointments and activities
