About The Position

We are seeking attentive and compassionate team members to join our team, and we provide two weeks of paid training. The Per-Diem position has monthly hourly requirements of at least 36 hours each month, at an hourly rate of $18.16. Direct Support Professional positions are responsible for maintaining an environment in the Day programs which encourages and facilitates optimal personal development and provides person directed services while ensuring the health and safety of the people supported.

Requirements

  • High School diploma, GED, or an acceptable TABE test score is required.
  • CPR/First Aid/AED and SCIP Certification within the first six months of hire.
  • Medication Administration Certification for Residential and site-based program, as required.
  • Non-site-based programs require one year of experience working with people with developmental disabilities or an Associate’s degree in Human Services or related field.
  • Valid driver’s license for the state in which the person resides.
  • Acceptable safe driving record.
  • Motor vehicle with up-to-date inspection, that is registered and insured.

Responsibilities

  • Ensure compliance with regulations, agency policies and procedures.
  • Ensure required documentation is complete, accurate and legible.
  • Ensure the agency’s incident management process is followed.
  • Provide input in the development and revisions of policies, procedures and agency forms as needed.
  • Ensure Chain of Command is followed when necessary and instructions are followed as stated in policy and procedures.
  • Assist with ensuring adherence to the annual budget.
  • Assist with ensuring accurate management of personal funds with the people receiving support.
  • Complete personal allowance documentation and other financial documentation according to procedure.
  • Provide services to people as planned in their Life Plan (LP).
  • Implement activities as outlined in the Staff Action Plan and, as applicable, the Life Plan.
  • Plan and coordinate daily activities.
  • Act as an advocate for people receiving support from the agency.
  • Provide person-directed services.
  • Complete assessments and compile baseline data as assigned.
  • Support people so they may be included and participate in their community.
  • Respect the person’s right to privacy and confidentiality.
  • Work collaboratively with others to ensure the person’s health and wellbeing.
  • Provide medication management and administer medications according to agency policy and procedures.
  • Ensure emergency medical procedures are followed and consult with the medical chain of command when necessary.
  • Report and document all medical concerns the person may have to the appropriate personnel.
  • Provide services and follow person’s Plan of Nursing Services (PONS).
  • Transport and attend health and medical appointments as assigned.
  • Maintain on-going communication with the Program Manager or Direct Support Professional V.
  • Maintain an environment which encourages and facilitates optimal personal development.
  • Complete assigned daily responsibilities.
  • Complete shift overlaps and specific duties as assigned.
  • Complete documentation, filing and general office maintenance and other routine operational reports.
  • Assist in the identification and development of new service and support opportunities.
  • Ensure the site is a safe, clean environment and immediately address areas of concern.
  • Maintain knowledge of fire evacuation plan and ensure plan is followed.
  • Maintain knowledge of health and safety plans and ensure plans are followed.
  • Practice standard precautions and other appropriate infection control practices.
  • Develop and maintain positive customer relationships both internal and external.
  • Ensure customer concerns are addressed in a timely and professional manner.
