Requirements

  • Support primarily the BHA homes, as a program and daily assignment locations will vary based on the needs of the program
  • support new and existing BHAs, providing support to adults with clinically enhanced support needs, supported by ICST and BHA, or CLA as needed, within their schedule
  • Ability to adapt to varying environments and needs
  • Strong Communication and interpersonal skills
  • Compassion, patience, and commitment to enhancing the quality of life for individuals in care
  • High School Diploma / GED
  • Two years of personal/professional experience working directly with adults with co-occurring diagnoses (dual diagnosis of autism/intellectual disabilities and mental/behavioral health diagnoses).
  • Working experience in crisis intervention, trauma-informed care and positive behavioral support approaches
  • Must have a valid PA/NJ Driver’s License and 1 year of driving experience

Responsibilities

  • Provides care and support to all individuals, maintaining therapeutic boundaries, to assure that they are safe and all living needs are met in accordance with the Individual Support Plan (ISP).
  • Implements and monitors each person’s ISP to achieve specific outcomes, derived from individual’s preferences, needs and interests within a trauma-informed recovery-oriented approach.
  • Implement medical and behavioral support plans as needed, using language and behavior that consistently reflects and enhances the dignity of people with dual diagnoses to support individuals to self-monitor triggers and utilize coping skills and problem solving skills.
  • Provide detailed, accurate and timely documentation for all services and supports provided.
  • Teaches and supports individuals in life skills to increase their independence and supports individuals to participate in the community including recreational activities and social events.
  • Supports individuals to develop self-directed goals, make informed choices and follow-through on responsibilities to experience positive self-image, hope and motivation.
  • Participates in assessments and mandated trainings to support the needs of the program and individuals being supported. 12. Assists individual to have the best possible health by teaching skills, providing supports and information on healthy lifestyles, promoting exercise, nutrition and sleep as outlets for stress management and communicating all health-related issues with SPIN Health Services.
  • Assists individual with maintaining a clean, healthy and safe environment and transportation needs to maximize skills, abilities, community inclusion and independence.
  • Performs emergency duties as requested in the best interest of SPIN and the health and welfare of its recipients and employees.
