About The Position

Maintains the general safety, security of consumers in crisis and aids consumers with the implementation of their individualized service plans. Essential Functions: Provides hands-on instruction to residents to implement crisis plans and assist with crisis resolution. As a member of the housing support team, provides hands-on support and assistance to Specialized Residential Unit II residents. Conducts recreational and social activities with residents, as assigned. Assists in meal preparation for the Specialized Residential Unit II residents. Provides transportation to medical appointments as necessary. Participates in regular staff meetings and trainings to develop basic knowledge and skills to assist residents in crisis resolution and support job performance. Maintains proper condition, cleanliness, and safety of the Specialized Residential Unit II, and responds appropriately to emergency situations. Other duties as required. Maintains proper documentation of all assistance and support provided to each resident. Adheres to all policies, laws, regulations and codes of ethics and confidentiality as outlined by federal and state laws and agency policies and procedures.

Requirements

  • High School Diploma, GED or an equivalent combination of skills, experience and education may be considered at the discretion of the Agency.
  • Ability to receive, understand and communicate information with effectiveness and follow instructions.
  • Must have a valid Pennsylvania Driver’s License and safe driving record.
  • Must have ability to utilize electronic medical records.
  • Must successfully obtain Criminal History Clearance from the PA State Police, Child Abuse Clearance from the PA Department of Human Services and FBI Fingerprint Clearance.
  • Must not be excluded from Medicare, Medicaid, or any other federal health care program.
  • Ability to transport oneself and/or individuals in an insured vehicle, as program needs dictate.
  • Ability to safely operate a variety of agency vehicles.
  • Ability to assist, move and/or transfer clients weighing up to 150 pounds, may be required.
