About The Position

The Direct Support Professional staff provides direct care services and supports to persons with intellectual and/or developmental disabilities. Duties consist of quality interaction and support of individuals in all activities of daily living, personal care, routine housekeeping, cooking, shopping, skills training and community integration. Settings in which services are provided include community (public) settings, individuals home, and center-operated Individual Skills and Socialization/Day Habilitation training sites. Performs moderately complex (journey-level) habilitative and rehabilitative care work with individuals who have developmental disabilities. Works under general supervision, with limited latitude for the use of initiative and independent judgment. The assigned shift will generally be M-F, 8am-4pm; however due to client needs, scheduled events, or staff outages there are times in which staff will be asked to provide services outside the typical M-F, 8-4. These events are typically known in advance and supervisor will work with staff to flex their schedule. If flexing is not able to occur, overtime pay will be given for hours exceeding 40 hours in a week (Sunday-Saturday). In the event a client service is scheduled on a center holiday, holiday pay will be given for time worked.

Requirements

High School diploma or GED.
Central Counties Services incorporates principles of trauma-informed care, which includes cultural sensitivity, in all areas of service delivery. All job descriptions include the responsibility to learn about and implement trauma-informed practices within the scope of work for the position.
Ability to read and interpret documents, such as policy and procedure manuals, Person Directed Plans and Implementation Plans.
Ability to communicate effectively orally, and written.
Ability to utilize the centers Electronic Health Record (EHR) to document services provided.
Ability to teach basic reading, math, time telling, writing, and cooking skills for training purposes. This list is not comprehensive and may require additional skills to delivery services based on individual need.
Ability to proficiently utilize computer programs, e.g., Microsoft Word and Excel.
Ability to see, hear and talk effectively within job requirements.
Ability to stand, sit, stoop, walk and drive for extended periods.
Ability to administer first aid or cardiopulmonary resuscitation (CPR) to conduct prevention and management of aggressive behavior (PMAB).
Ability to lift and move up to 100lbs with or without assistance.
Ability to remain calm in emergency/crisis situations.
Ability to perform tasks effectively and efficiently.
Ability to plan and schedule work and implement directives without constant supervision.
Ability to present a professional appearance and conduct when representing the Center.
Work behavior must be compatible with Center value statement and Policies and Procedures Manual contributing to an environment of problem solving, building trust, conflict resolution, and customer service.
Must remain actively engaged with individuals receiving services. For site-based services, staff seated independently at their desk or work station, should be limited. While in the community staff should remain nearby.
Ability to work cooperatively and productively with supervisor, individuals, co-workers, and groups of persons at all levels of activity, contributing to a strong spirit of teamwork.
Ability to establish and maintain effective working relationships with staff, other agencies and the general public using tact, courtesy and good judgment.
Must have and maintain a valid drivers’ license with an acceptable driving record.
Regular driving within the catchment area is required.
Must complete all center or state required trainings within the identified timeframe. Trainings must remain current.
Regular attendance and reliability is critical to business operations.

Responsibilities

Provides direct care, meaningful activities and skills training to individuals with intellectual and/or developmental disabilities as outlined in the Person Directed Plan, Individual Plan of Care and Implementation Plan. This may include assisting individuals with hygiene and personal care.
As a member of the service planning team, provides input regarding individuals service participation and progress towards goals.
Provides transportation for individuals based on identified service and support needs.
Provides direct supervision of self-administration of medications, or administers medication in accordance with Physician’s orders and as delegated by R.N.
Observes individual for signs and symptoms of disease, injury, reaction to medications, behavioral changes, or other conditions that warrant medical intervention. Report significant changes in individual’s physical conditions or behavior.
Utilizes approved techniques from Prevention and Management of Aggressive Behavior (PMAB) training to manage aggressive behavior among individuals, and provide intervention to ensure that individuals are protected from harm or injury.
Assists in the implementation of behavior support plan, as determined by Licensed Behavior Analyst. This may include completing behavior data cards and interventions specific to targeted behaviors.
Required to register as a provider in Texas Medicaid Electronic Visit Verification (EVV). Must complete EVV visit documentation through the EVV portal at the time of service delivery as required by Texas Medicaid.
Completes progress notes and service logs within timelines specified by division policy.
All documentation must meet billing guidelines for the individual’s specific program (ex. Home and Community-based Services, General Revenue, Texas Home Living, PASRR).
Completes incident reports within the time frames specified by the center.
Responds to inquiries or directives from division staff tasked with data management and quality assurance. All inquiries should be responded to within 1 business day, unless a shorter timeline is required due to state reporting of encounter data or billing. This includes data in both the center’s Electronic Health Records (EHR) and the Electronic Visit Verification (EVV).
Performs other duties as assigned.
Demonstrate high ethical standards, trustworthiness, and personal integrity. Regular attendance, dependability, and promptness are required for the work day hours 100% of the time, to ensure consistency and completeness of program’s processes.
Successfully complete training required to maintain skill competency and cross training to ensure consistency across program procedures. Complete continuing education as required by professional standards and the Center.
Frequent daily driving within the catchment area is required.
