About The Position

Under the direction of the HCS Program Manager and within federal, state and local guidelines provides entry level to routine habilitative and rehabilitative care work with individuals who have intellectual and/or developmental disabilities. The Direct Support Staff provides direct care and services to persons with developmental disabilities. The Direct Support Staff is available to the individuals served during their assigned shift to provide ongoing availability to the individuals served. This position will primarily provide support for individual’s medical appointments; however the duties of a Direct Support Professional can include support of individuals in all activities of daily living, personal care, routine housekeeping, cooking, shopping, and community integration. Works under moderate supervision, with limited latitude for the use of initiative and independent judgment. This position may be required to assist with community outings and may require work on Saturday and Sunday or outside of normal business hours to provide essential services to individuals who require services during these days/times. The general hours will be 8am-5pm or at the needs of the individuals.

Requirements

  • High School diploma or equivalent.
  • Ability to carry out program strategies as directed by license personnel.
  • Ability to liaison effectively with team regarding the status of a specific case.
  • Ability to respond calmly and quickly to crisis situations and maladaptive behavior.
  • Ability to see, hear and talk effectively within job requirements.
  • Ability to stand, sit, stoop, walk and drive for extended periods.
  • Ability to administer first aid or cardiopulmonary resuscitation (CPR) to conduct prevention and management of aggressive behavior (PMAB).
  • Ability to effectively handle potentially aggressive behavior in a positive manner.
  • Ability to proficiently utilize computer programs, e.g., Microsoft Word and Excel.
  • Ability to communicate effectively, both verbally and in writing.
  • Work behavior must be compatible with Center value statement and Policies and Procedures Manual contributing to an environment of problem solving, building trust, conflict resolution, and customer service.
  • Ability to perform tasks effectively and efficiently.
  • Ability to plan and schedule work and implement directives without constant supervision.
  • Ability to present a professional appearance and conduct when representing the Center.
  • Ability to work cooperatively and productively with supervisor, individuals, co-workers, and groups of persons at all levels of activity, contributing to a strong spirit of teamwork.

Responsibilities

  • Escorts individuals to medical appointments and ensures level of needed support is given based on individual needs. This may include transporting to/from appointments, checking-in, communicating with medical personnel.
  • Communicates with appropriate members of the individuals care team regarding medical appointments.
  • Provides direct care, meaningful activities and skills training to individuals with intellectual and/or developmental disabilities as outlined in the Person Directed Plan, Individual Plan of Care and Implementation Plan. This may include assisting individuals with hygiene and personal care.
  • As a member of the service planning team, provides input regarding individuals service participation and progress towards goals.
  • Assists in the implementation of behavior support plan, as determined by Licensed Behavior Analyst.
  • Provides direct supervision of self-administration of medications, or administers medication in accordance with Physician’s orders and as delegated by R.N.
  • Observes individual for signs and symptoms of disease, injury, reaction to medications, behavioral changes, or other conditions that warrant intervention. Report significant changes in individual’s physical conditions or behavior to provider nurse, behavior support specialist, program supervisor, or in emergency situations, 911.
  • Utilizes approved techniques from Prevention and Management of Aggressive Behavior (PMAB) training to manage aggressive behavior among individuals, and provide intervention to ensure that individuals are protected from harm or injury.
  • Required to register as a provider in Texas Medicaid Electronic Visit Verification (EVV). Must complete EVV visit documentation through the EVV portal at the time of service delivery as required by Texas Medicaid.
  • Completes progress notes and service logs within timelines specified by division policy.
  • All documentation must meet billing guidelines for the individual’s specific program (ex. Home and Community-based Services, General Revenue, Texas Home Living, PASRR).
  • Completes incident reports within the time frames specified by the center.
  • Responds to inquiries or directives from division staff tasked with data management and quality assurance. All inquiries should be responded to within 1 business day, unless a shorter timeline is required due to state reporting of encounter data or billing. This includes data in both the center’s Electronic Health Records (EHR) and the Electronic Visit Verification (EVV).
  • Performs other duties as assigned.
  • Demonstrate high ethical standards, trustworthiness, and personal integrity. Regular attendance, dependability, and promptness are required for the work day hours 100% of the time, to ensure consistency and completeness of program’s processes. Professional Development - Successfully complete training required to maintain skill competency and cross training to ensure consistency across program procedures. Complete continuing education as required by professional standards and the Center.
  • Frequent daily driving within the catchment area is required.
  • Staff member is expected to maintain their mental and emotional well-being as part of their role.
