About The Position

The Direct Service Worker (DSW) Supervisor/Facilitator is responsible for overseeing personal care assistants (PCAs) in a home care setting, ensuring quality care services are provided to clients. This role involves scheduling, supervising, and training PCAs, as well as maintaining client documentation and compliance with care plans. The supervisor will also engage with the community to promote services and ensure client satisfaction.

Requirements

• At least 7 years of experience working in the home care setting
• At least 3 years of supervisory experience with personal care in a home setting
• CNA/PCA/Medical Assistance Certification or willingness to obtain PCA Certification prior to hire
• Experience with the Medicaid Waiver Program
• Experience with hiring and supervising PCAs
• Experience working in an outpatient setting
• Experience visiting clients in the home and providing personal care and homemaking tasks
• Reliable, dependable, and highly organized
• Ability to work independently in the field

Nice To Haves

• Experience with LaSRS is a plus
• CPR / First Aid certified

Responsibilities

• Ensure clients are scheduled and seen by the PCA assigned
• Assignment of Clients to PCA as per Plan of Care
• Supervision of PCA's assigned
• Community awareness of services offered
• Monitoring of PCA paperwork for accuracy and quality
• Field Training of PCA's as assigned
• Act quickly and responsibly in cases of emergency
• Assure client/aide coverage
• Maintain client census and assist with increasing market share growth
• Scheduling of aides
• Assure required documentation is submitted timely

Benefits

• Full-time position
• Pay range of $15.00 - $18.00 per hour
• Expected hours: 40 per week
• Day shift schedule, Monday to Friday, with weekends as needed
