About The Position

4 2 Restore, LLC is seeking a compassionate and dedicated Direct Care Worker to provide support and assistance to individuals with mental health challenges in their homes and communities. The role involves delivering personalized care based on detailed client assessments, ensuring a safe environment, and promoting independence and self-advocacy among clients. Requirements

  • Associate's Degree or Bachelor's Degree in a Health-Related field
  • 2 years of work experience in Mental Health
  • 2 years of experience with ICANotes note writing skills
  • Submit to and pass a Pre-Employment drug test
  • Pass State required background screenings
  • Completed CPR/First Aid training
  • Possess a valid Driver's License in good standing
  • Reliable transportation and auto insurance that complies with the minimum state requirements
  • Ability to travel locally
  • Attend mandatory weekly & monthly meetings as required by the agency

  • Experience working with people with autism and developmental disabilities
  • Knowledge of home community care practices
  • Ability to lift and assist individuals as needed
  • Proficiency in meal preparation and household tasks
  • Strong patient care skills with a focus on empathy and understanding

Responsibilities

  • Work closely with individuals with mental health challenges to provide personalized care and support
  • Assist with activities of daily living, teach independence and self-advocacy
  • Ensure a safe and clean environment for individuals under your care
  • Develop and implement Treatment Plans and follow protocols to meet the specific needs of each individual
  • Help clients resolve problems, identify resources, and make effective use of resources
  • Provide companionship and emotional support to promote overall well-being
  • Provide a minimum of 6 or 4 hours of direct service, per client, per month
  • Prepare and maintain written case records, reports, notes, and other documentation in ICNotes
  • Adhere to HIPAA regulations and maintain confidentiality at all times
  • Perform administrative tasks as required
  • Participate in and complete all required training
  • Other duties as assigned
